The Hospice at Home team in central Norfolk provides short term enhanced palliative care to patients and their families when people who are coming to the end of their life need some extra support.
The service is available to patients with any life limiting condition including frailty.
Hospice at Home is a multi-disciplinary team from NCH&C which includes nurses, therapists and healthcare assistants who have enhanced knowledge and skills in palliative care and end of life care.
The service wraps around patients and links with the wider health and social care teams involved, allowing for greater integration and seamless care provision. Most are helped to remain at home in those last precious months, weeks and days.
The team also has the support of a dedicated voluntary and community sector coordinator from Community Action Norfolk, to ensure that patients and carers can be put in touch with local community support.
Family carers dealing with end of life situations are able to access a Carers’ Advice Line. The advice line offers information, advice and support for carers concerned about a loved one.
