In February 2019, North Norfolk Clinical Commissioning Group commissioned the Enhanced Care into Care Homes Service, delivered by North Norfolk Primary Care. North Norfolk Primary Care is an affiliation of the nineteen GP practices in North Norfolk.
The service aims to help people in care homes remain safe and well at home when a health problem arises, rather than have to go to hospital.
The Enhanced Care into Care Homes team is a small, multi-disciplinary team which includes advanced nurse practitioners, nurses and healthcare assistants, allocated to GP practices and the care homes patients they support. The service enhances the support already provided by the GP practices; whilst helping to free up GP time to focus on the highest risk patients.
Referrals into the service are via the GP practice, with each having a dedicated practitioner and healthcare assistant.
The response to the service has been extremely positive, with all GP practices and 90% of Care Homes already signed up in NN4.
A further element of this project will be to trial 20 Whzan devices. Whzan telehealth is applicable to a wide range of chronic and acute conditions and supports patients to gain a greater understanding of how to manage their own health.
