We are working together on how we can use health data to target our services more effectively. This is called Population Health Management.
This would mean looking at things like local statistics, local trends in people with different types of illnesses or anonymous data on admissions to hospital. From this we think we can identify how GPs, community physical and mental health teams and social care staff can work better in local areas.
We are not there yet - we have some really important issues to think about, such as what data we can use and whether it is telling us a full story. After all, we want our work to be based on good evidence.
Doctors in other areas of the country are thinking about population health management could work in their local areas.
